How inquiry and assessment information is handled.

Information collected

We may collect name, email, ZIP code, comments, care timing, assessment answers and result, the page where the visit began, referral source, campaign parameters, conversion event, timestamp, and browser user-agent information.

Do not submit diagnoses, medical records, Social Security numbers, insurance identifiers, payment information, or emergency details. The current forms are designed for non-medical care inquiries.

How information is used

Information may be used to respond to the inquiry, preserve the assessment context, route the lead to a provider, notify the selected provider, measure site and campaign performance, prevent duplicate or abusive submissions, improve site content, and maintain business records.

Provider sharing

When a visitor requests contact and accepts the form disclosure, the inquiry may be shared with appropriate providers for the stated location and needs. Providers handle information under their own privacy practices once received.

Analytics and advertising

Google Analytics 4 is used, with a visitor's permission, to understand visits, traffic sources, page use, and conversion events such as assessment progress and form submission. Google Analytics may use first-party cookies and receive page URLs and titles, on-site activity, approximate location, and browser or device information. Google states that IP addresses are used during collection and are not logged or stored.

Huntsville Care Guide does not intentionally send names, email addresses, ZIP codes, comments, or individual assessment answers to Google Analytics. Visitors can allow or decline Analytics when prompted and reopen that choice through the “Analytics choices” control in the site footer. Storage, security, and retention

Lead records are stored in a managed database. Reasonable technical safeguards are used, including validation, limited payload size, spam controls, and authenticated notification options. No system can promise absolute security. Records are retained only as long as reasonably necessary to respond, manage provider routing, prevent abuse, measure performance, maintain business records, and meet applicable obligations.

Your choices

You can use the public resources and complete the informational portion of the assessment without submitting contact details. To request access, correction, or deletion of a submitted inquiry, email contact@huntsvillecareguide.com.

Children and changes

This site is intended for adults making care decisions and is not directed to children. This notice may be updated when providers, analytics, notification services, or business practices change. The effective date will be revised.
